AAO Five Star Member Service Request

The AAO’s Five Star Member Service program is designed to recognize various occasions in members’ lives, careers and memberships. Please help us identify these occasions through your daily interaction with members and complete the following form to submit a request. Eligible occasions include:

  1. New baby (member) – available to domestic active members and their spouses who have a baby; recipient will receive a congratulatory card and AAO onesie
  2. New baby (BOT) – available to members of the BOT who have babies/grandbabies; recipient will receive a congratulatory card, AAO onesie and AAO bear/blanket
  3. Wedding – available to any domestic member who gets married; recipient will receive a congratulatory card
  4. New practice – available to domestic active members who open or purchase a practice for the first time; recipient will receive a congratulatory card and bundle of AAO store products (brochures, forms, etc.)
  5. Thinking of you – available to BOT and volunteer leaders who experience illness/injury/hospitalization; recipient will receive a gift basket
  6. Oops! – available to domestic members who have an unsatisfactory member experience; recipient will receive an apology card and Panera gift card (or similar depending on location)
  7. Thank you – available to any member who goes above and beyond to serve the association; recipient will receive a “thanks a latte” card and Starbucks gift card
  8. Condolences (member) – sympathy card sent to family of AAO members who pass away
  9. Condolences (member spouse/family) – sympathy card sent to AAO member who suffers a death in the family
  10. Condolences (AAO/ABO Board) – floral arrangement AND donation sent to family of former or current AAO or ABO Board members who pass away
  11. Condolences (volunteer leader) – floral arrangement OR donation sent to family of former or current volunteer leaders who pass away

Note: member birthdays, residency graduations and retirements are also recognized through this program as identified through AAO member records. It is not necessary to submit a request for these occasions.
